Project Brief
Horton PCN is a network of 10 GP practices working collaboratively across the Horton network area with community partners looking after the health and wellbeing of the local population.
Horton Primary Care Network is a key part of the NHS Long Term Plan, with ten general practices being a part of the network, covering 57,000 patients as of January 2021. The network will provide the structure and funding for services to be developed locally, in response to the needs of the patients they serve, The main purpose of Horton Primary Care Network is to improve the health of the public by providing easy access to medical care.
Their aim is to reform the local strategy in order to battle the health inequalities in the area but also to excel in the delivery of high-quality health and social care.
